Requirements to obtain a medical card in NY:
- The Green Nurse Intake Form
- The Green Nurse Disclosure, Waiver and Consent Form
- Provide one of the following to be uploaded to chart
1. Print off form from annual PCP visit stating diagnosis
2. Letter from PCP stating diagnosis
3. Letter from Therapist stating diagnosis
NOTE: The more details you provide on your intake form the more we will be able to help you.
Qualifying Conditions:
NOTE: These conditions must also be accompanied by one or more of the following associated or complicating conditions: cachexia or wasting syndrome, severe or chronic pain, severe nausea, seizures, or severe or persistent muscle spasms, PTSD or opioid use disorder.
- ALS
- Cancer
- Chronic pain – pain that degrades health and functional capability as an alternative to opioid use or substance use disorder.
- Opioid Replacement (if patient has been offered opiates and would prefer to try cannabis or wants to wean off opiates)
- Epilepsy
- HIV+/AIDS diagnosis
- Huntington’s disease
- Inflammatory bowel disease
- Neuropathy
- Parkinson’s disease
- PTSD, CPTSD which may include anxiety, depression, panic attacks, insomnia, agoraphobia

Process for NY
- The Green Nurse Consultation and card with NP
- Register with state and receive at temporary card
- Plastic permanent card sent in the mail
- NY patients can have up to 2 caregivers
